Is there a Security Audit?

Yes, Optimism has conducted many security audits, and has an open bug bounty of up to 2 million dollars for critical vulnerabilities.

Is it decentralized?

The sequencer is run by Conduit, but peer validation is permissionless. In the future, as OP Labs explores solutions for decentralization, we will integrate this functionality.

Can you include transactions without the sequencer?

Yes, you can post transactions on Ethereum Mainnet, and the sequencer is forced to pick these transactions up for inclusion on the next block.
